Incomprehensible logic

               Y created X.
               Y thinks that he/she is responsible with X.
               Z validated what was done by Y regarding X is correct.
               W said that something is wrong with X.
               Y thought that W is blaming him/her.
               W thinks that he/she simply stated a fact.
               W asked V if what he/she has said is perfectly fine.
               V answered yes.
               W now firmly believes that he/she is correct and that
                 he/she doesn't owe anyone an apology.

For the sake of X (which is between them), Y and W should talk alone. They should not involve any letter of the alphabet coz having too many variables will make things more complicated.
